ABOUT THE COMPREHENSIVE COMMUNITY SERVICES (CCS) OF JEFFERSON COUNTY
Jefferson County’s Comprehensive Community Services (CCS) program within the Human Services department provides a flexible array of individualized community-based psycho-social rehabilitation services for consumers with mental health or substance use disorders across their lifespan, with specific focus on children and adolescents. The goal of the program is to assist the consumer in increasing resilience, reducing the impact that their trauma history, mental health, and/or substance use disorder plays in their life, and assisting in their journey of recovery.

WHAT YOU WILL DO
The CCS team seeks a passionate and consumer focused individual to take on the Comprehensive Community Services (CCS) Service Facilitator I & II roles, primarily working with children and adolescents. These positions provide psycho-social rehabilitative services to consumers diagnosed with mental health and/or substance use disorders to reduce the negative impact of these disorders in their lives. 

CCS Service Facilitator I & II
You will be successful in this role by completing the following tasks and responsibilities:

  • Completes mental health assessments, including suicide and homicide assessments, collecting information about a consumer's past and current mental health and substance abuse issues, medical problems, and family and social interactions.
  • Develops recovery treatment plans for improving or eliminating the effects of mental health and/or substance abuse for the consumer.
  • Participates in team developed Recovery Planning Services, designed to provide the highest level of independent functioning and quality of life possible and desired by the consumer.
  • Facilitates and provides psychosocial rehabilitative services as specified in the Comprehensive Community Services Service Array.
  • Maintains knowledge of the applicable statutes, administrative codes, agency policies and procedures, administering mental health treatment per DHS 36 and 34. Adheres to the code of ethical standards.
  • Supervises court orders regarding mental health, guardianship, and protective placement.

 

CCS Service Facilitator I Requirements & Pay Range:

  • Bachelor’s degree in social work or related field from a college or university and one year experience working with individuals diagnosed with a mental illness and/or substance use disorder.

Pay Range: $27.77 - $35.72/ hour dependent on qualification.

CCS Service Facilitator II Requirements & Pay Range:

  • Master’s Degree in Social Work, Counseling or Marriage and Family Therapy from a college or university and one year experience working with individuals diagnosed with a mental illness and/or substance use disorder.
  • Ability to obtain one of the following professional licensures
    • Licensed Clinical Social Worker (LCSW)
    • Licensed Professional Counselor (LPC)
    • Licensed Marriage and Family Therapist (LMFT)

Pay Range: $32.12 - $41.26/ hour dependent on qualifications.
